We welcome referrals from support coordinators, plan managers, families, carers, hospitals, allied health professionals and participants themselves.
To make a referral, please contact us with the following details:
- Participant name and contact details
- NDIS number (if available)
- Supports required and preferred days/times
- Any risks, behaviours of concern or clinical needs we should know about
